Case Co-ordinator- Social CareDescription: We are recruiting for an experienced Case Co-ordinator to work alongside one of our clients a Local Authority in Lambeth. The ideal candidate must be Social Care Qualifies, hold a HCPC registration and current in date registered DBS check. Purpose of the role: * To undertake joint assessments on vulnerable users referred to the team, and to protect users and others in emergency situations. Taking into account the wishes of individuals and their carers. * To participate in risk assessments in relation to clients and others. * To manage a caseload and act as Care Co-ordinator for users on the CPA. * To jointly plan relevant responses to meet individual needs in consultation with all other interested parties. * To establish links with Children and Families, for the purpose of making referrals and attending case conferences regarding children who are potentially at risk. * To secure the delivery, within the financial resources available, of services to meet assessed needs. * To maintain an effective system for co-ordinating, monitoring and reviewing care plans meeting service standards; to identify gaps in services and to advise the line manager accordingly. * To maintain a range of costed resource information accessible to users, carers and other colleagues. * To maintain up-to-date case records in line with department standards. * Willingness to work flexibly according to service requirements. Additional Duties * To attend regular supervision, both professional and managerial. * To maintain links with professional colleagues and enable them to be involved in and make contributions to the assessment process. * To attend training, identifies within the appraisal process. * To keep abreast of legislation and departmental policies and procedures. * To carry out any other duties appropriate to the grade as may be required by the department. * To prepare all relevant reports required by the department, including mental health tribunal reports. * To ensure the service is delivered within the Council’s equal Opportunities and Employment Policy. * To make use of IT systems as required. This position is paying £19.35 PAYE per hour or £24.09 Limited Company per hour
