Care Manager - Learning Disabilities Team - South London- Social CareDescription: Care Manager - Learning Disabilities Team - South London We have a fantastic opportunity for a Care Manager to join the Learning Disabilities Team in South London. DUTIES To work as part of the Joint Community Learning Disability Team (JCLDT) to ensure the needs of clients who meet eligibility criteria are met in an appropriate way which promotes independence and well-being and achieves best value To be responsible for assessing individual adult’s needs, under the terms of the NHS and Community Care Act 1990, working jointly with health professionals and in liaison with service providers On the basis of assessed needs the Care Manager designs, arranges, develops and purchases individual packages of care - which are then monitored and reviewed REQUIREMENTS A Social Work or Learning Disability Nursing Qualification or equivalent, eg Dip SW or Combined Social Work and Nursing degree Working knowledge of relevant legislation which includes the NHS and Community Care Act 1990, various Carers’ Act legislation, the Mental Capacity Act 2005 and Mental health Act 1983 Knowledge of relevant guidance such as Valuing People and Fair Access to Care Working knowledge of the Benefits system Working knowledge of Safeguarding Adults processes and Child Protection Procedures Working knowledge of the impact of disability, especially learning disability, on the lives of individuals and their carers Ability to carry out detailed assessments of needs, including risk assessments, and develop robust care plans and support packages Ability to work effectively with clients, family carers, providers and other professionals Good negotiation skills including budgetary negotiations Ability to produce clear, succinct and analytical reports and other file recording Ability to use IT and maintain good electronic records with ability to learn the Council’s record keeping system Ability to chair meetings
