My client have a rare and exciting opportunity for a Care Coordinator to join their team to work at their well-established branch in London, (SW). The position: Working within a friendly yet busy office environment, you will be responsible for managing care hours for a case load of your own clients in and around the surrounding area. There will be many varying duties that you would be required to undertake including: To assist in the day to day coordination of the care hours including arranging for emergency cover when required, always ensuring continuity of care for the clients Allocating care and clinical staff & ensuring rotas are in order, completed and sent out on time Ensure records are kept up to date, including risk assessments and care plans Liaise regularly with clients and families to ensure the package is applicable and the client happy Provide written reports on complaints, accidents and incidents Provide excellent levels of customer service at all times Confirming details of carer visits to service users & issuing invoices accordingly Participating in the On-Call rota when required (Office Based)The Company: An established and highly reputable home care and nursing service provider based in London who specialise in providing home care to people with a range of different needs. This is a rare opportunity to join a fantastic company that supports its staff and really values its service users. The Person: You will ideally be qualified with an NVQ L2 or 3 in health & social care with experience of the care industry and working in a similar role. You will have experience of using Rostering Systems and will understand the scheduling process. The position will include on call duties on a rota basis. You will be personable, professional and be able to provide an excellent standard of customer care both over the phone and face to face. This position would be ideal for a care coordinator, zone manager, field supervisor, care giver, senior carer, deputy manager, registered manager, care manager, community care manager, home care manager, senior carer co-ordinator or team leader.
